What We Do

Care Management

Cozy Office

The Care Management team supports members with weekly check-ins, offering accountability, life skills, and emotional support. Each member receives a personalized action plan that helps them work toward goals in housing, employment, wellness, education, and independence.

Services We Offer

Couples Therapy

Mental Health Referrals

We connect individuals with trusted mental health providers for therapy, counseling, or psychiatric services based on need, location, and insurance status. 

Students Taking Exams

ELC Referrals

We provide support with Early Learning Coalition (ELC) referrals, helping families access affordable early childhood education and childcare programs.

Transportation Assistance

We help identify and secure transportation options such as bike programs, bus passes, or community transit services so members can get to appointments, work, or school reliably. 

Vision Exam

Vision Referrals

We assist with scheduling eye exams and connecting individuals to affordable vision care, including glasses or specialist services when needed. 

Doctor and Patient

Finding a Primary Health Provider

We help individuals find and establish care with a primary health provider for regular check-ups, preventative care, and ongoing health needs. 

Cars and Smiles

Transport

Transportation is provided within a set radius of our drop-in center for GED students, mental health appointments, and other discretionary requests. 

Nutritional Cooking

SNAP Assistance

Our team helps members apply for the Supplemental Nutrition Assistance Program (SNAP) to ensure access to healthy, affordable food and guides them through the application and eligibility process. 

Building Plans

Creating a Success Plan with Goals & Steps

Together with members, we develop personalized success plans that outline clear goals and practical steps to achieve stability and growth across multiple life areas. 

ID Tag Card

Vital Documents

We assist in obtaining vital documents, such as ID and DL, BC, SS cards. 

WAYS You Can Help

Navigation & Support Volunteers

Help members fill out and understand applications for SNAP, ELC, Medicaid, etc.

Life Skills Coach

Run small sessions on budgeting, cooking on SNAP, navigating healthcare systems, etc.

Health Literacy Volunteer

Teach clients how to understand prescriptions, ask questions at appointments, or interpret lab results.

  • 11 Members maintained WAYS housing for a full year.
  • 8 Members moved into permanent housing.
